Community Reablement Support Worker
We are seeking a compassionate and proactive support professional to help people regain independence and confidence after returning home from hospital.

Position: Community Reablement Support Worker
Location: Salisbury, Wiltshire
Salary: £29,979 to £31,458 per annum inclusive of 10% unsocial hours enhancement
Hours: Full time, 37 hours per week
Contract: Permanent

About the Role

An opportunity has arisen to join a community-based support team helping people safely transition from hospital back into their own homes. Working across the local community, you will provide practical and person-centred support to individuals recovering from illness, injury, or changes in circumstances, enabling them to regain confidence and independence.

This is a varied and rewarding role where no two days are the same. You will work closely with colleagues and healthcare professionals to assess needs, provide support, and develop personalised care plans that promote independence and wellbeing.

The role requires flexibility to work early mornings, evenings, weekends, and bank holidays as part of a rota.

Key responsibilities include:

  • Supporting individuals in their own homes following hospital discharge
  • Carrying out initial assessments and contributing to support planning
  • Providing personal care and assistance with daily living activities
  • Supporting individuals with meal preparation and maintaining independence
  • Working collaboratively with healthcare professionals and wider support teams
  • Maintaining accurate records and following safeguarding procedures
  • Delivering compassionate, respectful, and person-centred support at all times

About You

You will be caring, reliable, and passionate about supporting people to live independently. You will enjoy working both independently and as part of a team and be able to respond calmly to changing situations.

You will ideally have:

  • A Care Certificate, Level 2 Health and Social Care qualification, or equivalent experience
  • Experience supporting older people and/or people with learning disabilities
  • An understanding of safeguarding and community care principles
  • Good communication and interpersonal skills
  • Strong organisational skills and the ability to prioritise workload
  • A compassionate and empathetic approach
  • A flexible and adaptable attitude
